The doctor believes the infection can be treated, but the upcoming sequel finds him on a collision course with Sir Jimmy Crystal (Jack O’Connell) and his violent cult.

Filmed back-to-back with 28 Years Later, the fourth installment in the post-apocalyptic horror franchise hits theaters on January 16, 2026 via Sony.

Nia DaCosta (Candyman 2021, The Marvels) directs from a script by Alex Garland (28 Days Later, 28 Years Later).

Fiennes, O’Connell, Alfie Williams, Erin Kellyman, and Chi Lewis-Parry star, with Cillian Murphy reprising his 28 Days Later role.

In the film, Dr. Kelson finds himself in a shocking new relationship — with consequences that could change the world as they know it — and Spike’s encounter with Jimmy Crystal becomes a nightmare he can’t escape.

Danny Boyle produces alongside Garland, Andrew Macdonald, Peter Rice, and Bernard Bellew. Murphy executive produces.

The third chapter in the 28 Years Later trilogy was officially confirmed last month.
